Diverticulosis & Diverticulitis: What Every Patient Needs to Know

Introduction

Have you ever had a colonoscopy and been told, almost as an afterthought, that you have diverticulosis? Or have you ended up in the emergency room with sudden, sharp pain in your lower left abdomen and left with a diagnosis of diverticulitis? You are far from alone. These two conditions are among the top 50 most common diagnoses in all of medicine, and yet many patients leave the doctor's office without a clear picture of what is actually happening inside their colon.

This article walks you through everything you need to know: what these conditions are, who gets them, how they are diagnosed, how they are treated, and most importantly, what you can do right now to protect yourself.

What Is Diverticulosis — And How Does It Become Diverticulitis?

Think of your colon as a long, flexible garden hose. Over time — particularly with a low-fiber, Western-style diet — small, grape-sized pouches called diverticula can form by bulging outward through weak spots in the colon wall. When these pouches are simply present without any trouble, the condition is called diverticulosis. Most people never know they have it.

Diverticulosis: Pouches are present but not inflamed. Usually no symptoms. Very common, especially with age.

Diverticulitis: One or more pouches become inflamed or infected, causing pain, fever, and other symptoms. Think of it as the pouches "getting angry."

A helpful analogy: diverticulosis is like having potholes in a road — they exist but do not cause problems until something gets lodged in them and causes damage.

Epidemiology: Who Gets Diverticular Disease?

Diverticulosis affects approximately 10% of adults under age 40, rising to 60% of adults by age 60, and over 70% after age 80.


Only about 4% of people with diverticulosis will develop diverticulitis — but that still translates to over 300,000 hospitalizations in the United States every year.


Historically considered a disease of older adults, diverticulitis is now increasingly diagnosed in younger adults aged 18–44, likely driven by obesity, sedentary lifestyles, and low-fiber diets.


Diverticular disease is far more common in Western countries (USA, Europe, Australia) compared to Asia and Africa — a pattern strongly linked to dietary fiber intake. Men under 50 develop diverticulitis more often than women; after 50, the rates equalize.

How It Presents: Signs and Symptoms

Diverticulosis: Usually completely silent. Some people notice occasional mild cramping or bloating, but most have no symptoms at all and only discover it during a routine colonoscopy. Diverticulitis symptoms typically develop over several hours and include:

  • Lower Left Abdominal Pain: The hallmark symptom — a constant, dull-to-sharp ache in the lower left side of the abdomen. The pain often worsens with movement.

  • Fever and Chills: Low-grade to high fever signals active infection and inflammation.

  • Nausea and Vomiting: Common, especially in more severe cases.

  • Changes in Bowel Habits: Either constipation or, less commonly, diarrhea.

  • Bloating and Distension: The abdomen may feel full and tender to the touch.

  • Blood in Stool: Uncommon with diverticulitis itself, but diverticulosis can cause painless rectal bleeding — a separate complication called diverticular bleeding.

  • Warning Signs Requiring Emergency Care: Severe or worsening pain, high fever above 101°F (38.3°C), inability to keep fluids down, or significant rectal bleeding all require immediate medical attention.

Diagnostic Tests and Imaging

CT Scan with or without IV Contrast — The Gold Standard: This is the most important test for diagnosing diverticulitis. It achieves a sensitivity of approximately 97% and a specificity approaching 100% for confirming an inflamed diverticulum. It also identifies complications such as abscesses, perforation, or fistula.

  • Blood Tests: A Complete Blood Count (CBC) will typically show an elevated white blood cell (WBC) count, indicating infection. C-reactive protein (CRP), an inflammation marker, is also elevated and helps gauge severity.

  • Urinalysis: Because diverticulitis pain can mimic a urinary tract infection (UTI) or kidney stone, a urinalysis is often ordered to rule these out.

  • Colonoscopy: Colonoscopy is NOT performed during an acute attack — the pressure of the scope on an inflamed colon risks perforation. However, it is strongly recommended 6–8 weeks after recovery to rule out colon cancer, which can mimic diverticulitis on imaging.

  • Ultrasound: Useful when CT is unavailable or in pregnant patients. Lower sensitivity (~84%) than CT, but avoids radiation exposure.

The Hinchey Classification — How Doctors Grade Severity:

  • Stage 0/Ia: Mild inflammation confined to the colon wall. Treated as an outpatient.

  • Stage Ib: Small pericolic abscess (pocket of pus near the colon). Usually requires hospitalization and IV antibiotics.

  • Stage II: Larger pelvic or intra-abdominal abscess. Often requires CT-guided drainage.

  • Stage III: Purulent (pus-filled) peritonitis — infection has spread to the abdominal cavity. Surgical emergency.

  • Stage IV: Fecal peritonitis — bowel perforation has contaminated the abdominal cavity with stool. Surgical emergency.

How to Treat Diverticulitis

Treatment depends on severity. The vast majority of cases (approximately 75%) are uncomplicated and can be managed at home.

  • Uncomplicated Diverticulitis (Home Management):

  • Diet: Clear liquid diet for 1–3 days to rest the colon (broth, water, juice without pulp, popsicles), then gradually advance to soft foods, and then a normal diet over 1–2 weeks.

  • Antibiotics: Oral antibiotics such as ciprofloxacin plus metronidazole, or amoxicillin-clavulanate for 7–10 days. Recent research suggests antibiotics may not always be necessary for very mild uncomplicated cases — discuss this with your doctor.

  • Pain Management: Acetaminophen (Tylenol) is preferred. Avoid NSAIDs (ibuprofen, naproxen, aspirin) — they may worsen inflammation and increase the risk of perforation.

  • Rest and Hydration: Drink plenty of fluids throughout recovery.


Complicated Diverticulitis (Hospital Management):

  • Intravenous (IV) antibiotics and IV fluids are the foundation of hospital treatment.

  • CT-guided percutaneous drainage for abscesses larger than 4–5 cm.

  • Surgery (bowel resection) is reserved for: perforation with peritonitis, failure to respond to antibiotics, fistula formation, or repeated severe attacks.

Important Note on Recurrence: After a first episode of uncomplicated diverticulitis, approximately 20–40% of patients will have a recurrent episode. Surgery may be discussed after multiple recurrences, but the decision is always individualized.

How to Prevent Diverticulitis

Prevention is powerful here. Research consistently shows that lifestyle choices significantly affect your risk of both developing diverticulitis and preventing recurrence.

1. Eat More Fiber: Aim for 25–38 grams of dietary fiber per day. Good sources include fruits (apples, pears, berries), vegetables (broccoli, Brussels sprouts, carrots), whole grains (oats, brown rice, whole wheat bread), and legumes (beans, lentils).

2. Stay Hydrated: Drink 6–8 glasses of water daily. Fiber works best with adequate hydration.

3. Exercise Regularly: Research recommends at least 2 hours of vigorous physical activity per week. Exercise reduces intra-abdominal pressure and promotes healthy gut motility.

4. Maintain a Healthy Weight: Obesity — particularly central (belly) fat — is a significant risk factor. Each unit increase in BMI is associated with increased diverticulitis risk.

5. Limit Red Meat: Keeping unprocessed red meat intake below 51 grams per day (roughly 1.8 oz) is associated with reduced risk.

6. Avoid NSAIDs When Possible: Regular use of ibuprofen, naproxen, and similar drugs is associated with a higher risk of diverticular complications.

7. Quit Smoking: Smoking is a recognized risk factor for diverticular complications.

8. The Seeds and Nuts Myth — DEBUNKED: For decades, patients were told to avoid seeds, nuts, corn, and popcorn. Multiple large studies have now confirmed this advice was incorrect. There is no evidence these foods trigger diverticulitis, and they can be eaten freely.

Living With Diverticulosis: What to Expect Long-Term

A diverticulosis diagnosis does not have to be frightening. The overwhelming majority of people with diverticulosis (96%) never develop diverticulitis. Think of your diverticulosis diagnosis as a motivating reason to adopt the preventive lifestyle habits above — many of which benefit your heart, brain, and overall longevity as well.


After a diverticulitis episode, follow up with your gastroenterologist. A colonoscopy 6–8 weeks after recovery is standard practice to ensure there is no underlying colorectal cancer. Long-term, maintaining a high-fiber diet and healthy weight are your best tools.
